A thorough review of the provided medical records will be submitted via a secure portal. Physicians should obtain patient’s consent prior to submitting records using their office’s standard medical release form.
Intake Questionnaire
Fill out of intake questionnaire to assist us in providing the answers you need. Specific questions can be asked or a basic review of the case can be completed. The more information you provide the more comprehensive our recommendations can be.
Video/Phone Conference
A scheduled video/phone conference to further discuss the case along with a review of findings and recommendations.
Detailed Recommendations Report
A comprehensive report of key findings, differentials, recommendations for therapeutic options, or additional testing will be submitted back to you.
Follow-Up
Each review will include an optional follow-up 15-minute call to discuss outcomes or additional recommendations.
